Care is available for patients that need help at home after being discharged from the hospital or after an outpatient procedure. An aide can provide the short-term assistance needed to:

• Accompany the client home from medical treatment*
• Help the person get comfortable back at home
• Shop
• Pick up medications or supplies
• Prepare a meal and tidy up

*With advanced notice, prior arrangements can be made to have an aide drive a client — as long as the client's own car is used.

Longer term assistance can also be arranged by speaking with one of our Intake Coordinators.
